A Personal Approach to Care
Our philosophy is to let the patient be our guide, helping our skilled specialists design an individualized treatment plan to meet the patient’s specific needs and goals. Each patient is cared for by an interdisciplinary treatment team under the direction of a physician specializing in orthopedic rehabilitation. Working collaboratively towards the patient’s goals, the team consists of rehabilitation nurses, physical and occupational therapists, case managers and other professionals. This dedicated team of clinicians specialize in orthopedics and holds certifications and specialties in orthopedic rehabilitation. Family members are encouraged to participate in the rehabilitation process.

Admissions
Depending on a patient’s medical condition, surgical status and age, they may be admitted to HHH’s disability-specific acute or sub-acute rehabilitation unit. The rehabilitation length of stay for single or bilateral joint replacement or fracture ranges from one to two weeks.

Customized Treatment Plan
The referring physician’s protocols concerning weight bearing, medication and therapies are precisely followed and form the basis of the treatment plan, which may include:

  • Seven or five day a week therapy (dependent on acute or sub-acute unit)
  • Individual & group therapy sessions utilizing NuStep, CPM & other equipment as indicated
  • Physical Therapy – restores mobility, endurance, strength and range of motion
  • Occupational Therapy – focuses on activities of daily living, including self-care and home & food management skills
  • Pain management
  • Medication evaluation & optimization
  • Patient/family education
  • Case management/discharge planning, including ordering of mobility and adaptive devices
  • Individual home exercise program

Follow-up Care
Following inpatient rehabilitation, patients may benefit from HHH’s full continuum of care, including:

  • Outpatient Physical & Occupational Therapy
  • Aquatic (warm water pool) Therapy
  • Driver Evaluation & Training
  • Osteoporosis Center
  • Prosthetic Orthotic Center
  • Wellness Center
