The Day Hospital offers an innovative option to shortened
hospital stays, bridging the gap between traditional inpatient,
home care and outpatient rehabilitation programs. The program
offers a smooth continuum of comprehensive and intensive medical,
nursing and therapeutic services in a supportive and cost
effective environment. Patients have the advantage of living
at home while receiving fully integrated services focused
on promoting maximum independence and well being.
Who can benefit
The Day Hospital is designed for patients who
are medically stable and able to be transported to and from
the program. Admission can be initiated at any point in the
recovery process, such as immediately following an acute hospital
stay or directly from home, soon after or several years post-injury.
The Day Hospital may also be recommended for individuals who
have progressed well in acute rehabilitation and can benefit
from continued, intensive therapy, but no longer require 24-hour
care. The program is appropriate for individuals with:
· brain injury*
· spinal cord injury
· cognitive disorders
· stroke
· multiple sclerosis
· neurological conditions such a
hALS and Guillain-Barre Syndrome
· multiple trauma
*Brain injury could be the result of trauma, congenital disorder, tumor anoxia, infection or hypo perfusion.
Treatment plan and goals
Following a thorough evaluation by a team consisting of a nurse practitioner, case managers, nurses and therapists, a treatment
plan will be customized to meet a patient's needs and goals.
Most individuals require a combination of physical therapy,
occupational therapy, speech therapy and skilled nursing care.
In addition, the services of neuropsychologist, psychiatrist,
vocational counselor, dietitian and recreational therapy
specialist are available as needed, depending on the limitations and exclusions of the patient's insurance contract.
Day Hospital patients also have access to the
many specialized services of Helen Hayes Hospital, including
the Prosthetic Orthotic
Center, Center for
Rehabilitation Technology, videoflouroscopy, driver
evaluation, vestibular therapy, and aquatic
therapy.
Families and caregivers are encouraged to play
an active role in the rehabilitation program. They are provided
with ongoing support and education to ensure that skills and
strategies developed in the Day Hospital are integrated into
daily activities at home and in the community. Patients participate
in the program on a two, three or five day a week schedule,
according to their medical and rehabilitation needs.
Coverage
Treatment is covered
by Medicare, Medicaid and most insurances. The Day Hospital case managers will, to the best of their abilities, assist the patient/family with understanding the limits of the insurance benefits so they can make an informed decision about participation in the program.
Referrals
The Helen Hayes Day Hospital accepts referrals from physicians, case managers, health care professionals and from the patient or family directly.
